The polio finding in the UK has surprised some of my colleagues — people think of polio as a disease of the developing world. But London's wastewater surveillance has been picking up poliovirus for multiple years now, primarily from unvaccinated immigrant communities in east London, and the UK Health Security Agency has confirmed paralytic cases in children. I bring this up because a lot of American patients think of European travel as vaccine-free — it's not, and this one actually matters.

The CDC recommendation now is an IPV booster for anyone traveling to the UK or Germany who hasn't had one as an adult. Beyond the polio issue, the UK is genuinely one of the safest travel destinations in the world from a health standpoint. The NHS is world-class.

The food is safe. The tap water is safe. The main clinical things I think about are seasonal influenza (particularly in winter), confirming MMR status given the global measles resurgence, and making sure travelers know that an NHS emergency visit will cost them real money without insurance.

Folks come back with bills for several thousand pounds because they didn't have travel insurance.
