The UAE — normally one of the world's most medically sophisticated and clinically low-risk travel destinations — is currently operating under a Level 3 State Department advisory due to the active US-Iran conflict that began February 28 2026. Iran has publicly stated its intent to target UAE locations associated with the United States; the Embassy and Consulate are closed and all routine consular services are suspended; the US Mission has issued shelter-in-place guidance to remaining personnel. From a health standpoint the UAE has no malaria; no yellow fever requirement; and world-class hospitals including the JCI-accredited Cleveland Clinic Abu Dhabi — but the dominant risk right now is geopolitical violence; not infectious disease. The single most important clinical preparation for anyone still in the UAE is a 72-hour supply of all medications and knowledge of the nearest hospital and bomb shelter.

Armed Conflict and Missile/Drone Threat
The dominant threat in UAE as of March 2026 is active armed conflict following the onset of US-Iran hostilities February 28 2026. Iran has publicly and specifically stated its intent to target UAE locations associated with the United States. The FAA has issued a NOTAM for the region. US government personnel have been ordered to depart; the Embassy and Consulate are closed. Houthi rebels struck Abu Dhabi in January 2022 with drone and missile attacks causing civilian fatalities and fires at the Abu Dhabi National Oil Company and Abu Dhabi International Airport. The current threat environment significantly exceeds that 2022 baseline.

Exit Ban and Legal Risk
UAE enforces strict laws on social behavior; drug possession (including trace amounts of codeine and some anxiety medications legally prescribed in the US); financial crimes; and traffic violations. Violations can result in immediate arrest and exit bans that prevent departure indefinitely with no advance notice. Some US prescription medications are outright illegal in UAE — verify every medication with the UAE Embassy before travel. Exit bans are a well-documented risk for both business travelers and tourists. In the current conflict environment with a closed US Embassy; an exit ban is an exponentially more dangerous situation because the US government has no ability to provide in-person consular assistance.

MERS-CoV Exposure Risk
Middle East Respiratory Syndrome coronavirus (MERS-CoV) is a zoonotic virus transmitted from dromedary camels to humans. The UAE has reported 3 MERS-CoV human cases since 2019. No vaccine; no antiviral; supportive care only. Case fatality rate historically 35%. Risk to typical tourists is very low. Avoid all direct contact with dromedary camels; do not drink raw camel milk or camel urine; do not eat improperly cooked camel meat. Returning UAE travelers who develop fever with pneumonia or ARDS within 14 days require MERS-CoV evaluation.
Health Risk Callout
No malaria; no yellow fever; world-class hospitals; safe food. That picture is unrecognizable right now. The clinical consultation for UAE in March 2026 is about trauma preparedness; medication security; and exit planning. Cleveland Clinic Abu Dhabi is genuinely one of the world's finest hospitals — and getting there when missiles are incoming is a different calculation than under normal circumstances. UAE has extremely strict drug laws (some US prescription medications are illegal — verify all medications before entry); exit bans (documented risk for both tourists and business travelers); photography restrictions (never photograph government buildings; military installations; or individuals without consent); LGBTQ+ relationships are illegal with criminal penalties; alcohol is legal only in licensed establishments; dress codes in public spaces outside resort areas; heat illness risk (summer temperatures exceed 45°C — extreme outdoor activity June-September is dangerous)

The UAE's geographic risk profile has been fundamentally altered by the February 28 2026 US-Iran conflict. Dubai and Abu Dhabi — the country's two primary cities and normally its safest — are the highest-risk zones because they contain the US-associated locations Iran has specifically threatened. The US Mission has issued shelter-in-place guidance. Remote emirates such as Fujairah or Ras Al Khaimah have lower US-association visibility but no meaningful safety advantage in an active missile threat environment. The FAA NOTAM covers all UAE airspace. Houthi rebel groups have previously struck Abu Dhabi in January 2022 causing civilian fatalities; the current Iran-US conflict has dramatically elevated this threat class.
Regional Healthcare Callout
Dubai and Abu Dhabi have multiple JCI-accredited world-class hospitals that under normal circumstances represent some of the finest medical infrastructure in the world. The current armed conflict creates genuine uncertainty about sustained operations. American civilians requiring emergency medical care must access hospitals directly — no US Embassy or Consulate is available. Medical evacuation to Europe during active conflict is severely compromised. All serious medical situations must be managed locally.

The UAE is where the travel medicine consultation has become a different kind of conversation entirely. I'll always associate this place with extraordinary clinical infrastructure — Cleveland Clinic Abu Dhabi represents some of the finest medicine anywhere in the world. The pre-travel visit for UAE used to be brief and pleasant: no malaria; routine vaccines; a quick word about the drug laws and the heat.
Right now the conversation is about conflict preparedness and evacuation. The most important clinical point for anyone still in the UAE is the medication supply issue — UAE drug laws are genuinely strict; some medications that are completely standard in the US are criminal offenses there; and with the Embassy closed; there is no consular rescue if you are arrested. Maintain 72 hours of medications; know your hospital; know your bomb shelter; call the State Dept task force; and leave if there is a safe way to do so.
The excellent medical system is still there — Cleveland Clinic Abu Dhabi does not close during a regional conflict. But access to it under active missile threat conditions is a different clinical calculation than it was in January.
