Ukraine is a Level 4 dossier, and I write these notes for the journalists, aid workers, and NGO staff who will go anyway — as preparation, not encouragement. The defining clinical fact of this war is antimicrobial resistance: in a CDC-supported survey of Ukrainian regional hospitals, 60% of healthcare-associated infections involved a carbapenem-resistant organism, and every Klebsiella pneumoniae isolate tested was resistant to carbapenems — the last-line antibiotics. Any wound treated in-country must be assumed colonized; European hospitals now screen and isolate patients evacuated from Ukraine. Meticulous wound care and early ground evacuation to Poland are part of the medical plan, not afterthoughts.
Beyond trauma: WHO reports roughly 1,000 measles cases in the first five months of 2026 — confirm two documented MMR doses. Tick-borne encephalitis foci sit precisely in the western oblasts where humanitarian teams base — Volyn, Rivne, Zhytomyr, the Carpathians — so TICOVAC belongs in the 8-week conversation, alongside rabies pre-exposure: CDC surveillance documents rabies reemerging across the region as wartime disruption undermines wildlife vaccination.

Pre-Departure Checklist

- DO NOT TRAVEL HERE — Ukraine is Level 4 with active armed conflict. Confirm whether the trip should proceed at all
- Dual citizens: males aged 18–60 must consult the Ukrainian Embassy regarding mobilization before travel
- CDC recommends routine vaccines including MMR
- Hepatitis A
- Hepatitis B
- TBE vaccine (Ukraine is TBE endemic — the primary series needs lead time)
- Rabies pre-exposure (rabies reemerging across Central Europe — CDC EID Feb 2026).
2–4 Weeks
- Register with STEP and notify KyivACS@state.gov
- Confirm war-zone insurance — standard policies exclude active conflict
- Download and configure the Air Raid Siren app BEFORE crossing the border
- Plan a ground evacuation route to Poland or Romania
- Carry a 30-day medication supply.
