Turkmenistan is the consultation where logistics are the medicine. The letter of invitation means nearly every traveler arrives through a tour operator — so make the operator part of your medical plan: put evacuation procedures, vehicle backup in the desert, and communication protocols into the contract before you go. The State Department is unusually blunt about why: ambulances here are not staffed with trained paramedics and often carry little or no equipment, dispatchers speak Turkmen or Russian, and payment is cash at time of service. Bring your entire medication supply from home in original packaging — many Western drugs simply do not exist here — and carry medevac coverage that reaches Istanbul, four hours out, or Dubai, three.

The clinical prep itself is modest: hepatitis A and typhoid for variable food and water hygiene, routine vaccines current, bottled water throughout. The environment does the rest of the talking. Darvaza means desert camping in the Karakum, where summer temperatures routinely pass 113°F — schedule activity for morning and evening, carry more water than feels reasonable, and treat confusion or cessation of sweating as an emergency. Ashgabat sits in an active seismic zone, and one practical note travelers miss: the emergency number is 03, and the response may be a very long time coming.
