Health Overview
Taiwan is one of Asia's most rewarding and safest travel destinations — a modern democracy with world-class universal healthcare; outstanding food culture; stunning natural landscapes; and a Level 1 State Department advisory. The pre-travel consultation is brief: routine vaccines; no malaria; no yellow fever; generally safe tap water in Taipei and major cities. The primary clinical and safety considerations are natural disaster risk (Taiwan sits on a major seismic fault; experienced a 7.4 magnitude earthquake on April 3; 2024; and averages 5 typhoon warnings per year) and dengue fever in southern Taiwan (Kaohsiung; Tainan; and surrounding areas). Taiwan has one of Asia's best-regarded hospital systems — National Taiwan University Hospital consistently ranks among Asia's finest academic medical centers. Note: The US does not have a formal embassy in Taiwan; services are provided through the American Institute in Taiwan (AIT).

Dengue Fever
Dengue is endemic in Taiwan with periodic outbreaks particularly in Kaohsiung; Tainan; and southern Taiwan. The 2014 dengue outbreak in Kaohsiung was devastating (15,000+ cases). DEET is recommended for southern Taiwan travelers; particularly June through November. Taipei and northern Taiwan have lower dengue risk but not zero — local transmission has occurred in the capital during outbreak years. All four dengue serotypes circulate. DEET 20%+ is the standard recommendation for any outdoor activity in Taiwan.

Seismic and Typhoon Risk
Taiwan is seismically one of the world's most active areas — the April 3; 2024 magnitude 7.4 earthquake was the strongest in 25 years; killing 13 and injuring hundreds; and causing major damage in Hualien and the east coast. Average of 5 typhoon warnings per year with 3-4 typhoons making landfall; typhoon season May-November. Travelers should know their hotel's earthquake assembly point; keep shoes beside the bed (to walk through debris); and monitor the Taiwan Central Weather Administration for typhoon warnings.

Travelers' diarrhea risk in Taiwan is low — the country has excellent food safety standards; cold chain management; and sanitation infrastructure comparable to Western Europe. The famous night market and street food scene is generally safe due to high turnover and established vendors. Standard food hygiene precautions apply; avoid raw shellfish and questionable ice.

The seismic situation in Taiwan deserves specific patient counseling that most travel medicine clinicians skip. I tell every Taiwan traveler to find the earthquake assembly point at their hotel on check-in — this takes 60 seconds and could save their life. Hualien is a beautiful destination but it was the epicenter of a magnitude 7.4 quake in April 2024; and it will have future earthquakes. This is simply the geology of Taiwan's location on the Philippine-Eurasian plate boundary. Knowing what to do in the first 60 seconds of a major earthquake — under a table; away from windows; move outside only after shaking stops — is the key.

Regional Risk Callout
Taiwan's risk profile is remarkably uniform across the island for health purposes. Taipei; Taichung; and northern Taiwan are very low risk for dengue. Southern Taiwan — particularly Kaohsiung and Tainan — carries active dengue transmission particularly June through November. Eastern Taiwan (Hualien; Taroko Gorge) is seismically the most active zone — Hualien was the epicenter of the devastating April 3; 2024 magnitude 7.4 earthquake. Mountain trekking in Taiwan's Central Mountain Range reaches elevations over 3,000–3,900m (Jade Mountain at 3,952m) — altitude illness becomes relevant for rapid ascent trekking. The Penghu Islands and outlying islands have standard Taiwan health risk.
Regional Healthcare Callout
Taiwan's hospital system is one of the few in Asia where the public system is genuinely world-class. National Taiwan University Hospital; Taipei Veterans General; MacKay Memorial; and the major regional medical centers in Taichung; Tainan; and Kaohsiung all provide excellent care at low cost to all comers (including foreigners who can pay fees). For travelers injured in an earthquake or typhoon — even in remote eastern Taiwan — helicopter evacuation to Hualien Tzu Chi Hospital (eastern Taiwan) or to Taipei is the standard. Medical evacuation to the US is rarely necessary from Taiwan.

Taiwan is the cleanest pre-travel consultation No malaria; no yellow fever; world-class hospitals; safe tap water; safe food culture; Level 1 advisory. The main things I cover are dengue (DEET is non-negotiable for anyone visiting Kaohsiung or Tainan during summer and fall) and earthquakes. The seismic conversation is actually the most important safety point for Taiwan — not because the risk is extreme; but because the April 2024 earthquake showed that Taiwan is actively seismically alive; and knowing what to do in the first 60 seconds of a major earthquake is a piece of information that takes two minutes to transmit and could genuinely save a patient's life.
NTUH is one of the best hospitals I'd want to be a patient in. Taiwan's universal NHI system means that foreigners presenting at virtually any hospital in Taiwan will receive world-class care at a fraction of what the same service would cost in the US. Medical tourism to Taiwan is an underappreciated option for elective procedures.

Taiwan has excellent medical infrastructure capable of managing virtually all acute and complex medical conditions. Medical evacuation to the US is rarely medically necessary from Taipei. Note that the US does not maintain a formal embassy in Taiwan — the American Institute in Taiwan (AIT) performs equivalent services for US citizens. Emergency AIT number is +886-2-2162-2000 (24/7). Medical evacuation insurance with earthquake/typhoon natural disaster coverage is advisable.
