Vaccine and Malaria Prevention

- Areas: Present in some parts of Myanmar throughout the country. Most risk in rural and forested areas. Higher risk in: Kachin State; Kayah State; Kayin State; Chin State; Shan State; Rakhine State; Mon State; Tanintharyi Region; Bago Region (rural areas). Lower risk in: Yangon; Mandalay city; Naypyidaw; Sagaing city proper. P. falciparum (primarily); P. malariae; P. ovale; P. vivax (less common).
- Recommended Drug: Atovaquone-proguanil; Doxycycline; or Tafenoquine for eastern border areas with confirmed mefloquine resistance (Shan State border with China and Laos). Chloroquine not recommended. Mefloquine NOT recommended for eastern states bordering China and Laos due to confirmed resistance.
- Drug Resistance: Yes
Mefloquine-resistant P. falciparum is confirmed in Myanmar's eastern states on the border with China and along borders with Laos — this is the same resistance pattern documented in Mekong Delta Southeast Asia. Atovaquone-proguanil; doxycycline; or tafenoquine are the correct prophylaxis agents for any Myanmar itinerary; mefloquine should not be prescribed. Tafenoquine requires G6PD testing before prescription. Treatment in Myanmar outside of Yangon is unreliable — travelers should carry their prophylaxis plus a course of artemisinin-based combination therapy (ACT) as a self-treatment pack if traveling to areas without reliable medical access. P. vivax-endemic areas additionally require radical cure consideration with primaquine or tafenoquine after return (G6PD testing required).
Physician Note
Myanmar is the hardest clinical conversation in this batch — because the question is not 'which vaccines does this person need' but 'should this person go at all.' DO NOT GO. The Level 4 advisory exists because the military regime has shown a documented pattern of wrongfully detaining US nationals; an active civil war has made large portions of the country genuinely inaccessible; the March 2025 M7.7 earthquake compounded an already catastrophic healthcare infrastructure collapse; and mefloquine-resistant malaria in the east adds a specific drug resistance nuance that distinguishes Myanmar from a standard Southeast Asia malaria discussion. For anyone who has a compelling reason to go — the yellow fever entry requirement is universal and non-negotiable; the correct malaria prophylaxis is atovaquone-proguanil or doxycycline; rabies pre-exposure is strongly indicated given the absence of reliable post-exposure treatment outside Yangon; and medical evacuation insurance that explicitly covers Myanmar is mandatory.
The clinical work here is as much about evacuation planning as it is about vaccination.
