Lake Malawi is one of the most significant schistosomiasis exposure sites in southern Africa. S. mansoni and S. haematobium both present. Entire lake should be considered risk water. The correct clinical answer is NOT 'don't swim' but 'swim with informed consent and plan for praziquantel 6-8 weeks post-return.'
