Active Armed Conflict and Rocket/Missile Threat
Israel has been under sustained rocket; missile; and armed UAV threat since October 7 2023. The security environment described by the State Department as 'unpredictable' with rocket and missile attacks occurring without warning in Tel Aviv; Jerusalem; and throughout the country. US government authorized departure was issued February 27 2026. The Red Alert app (Home Front Command) is the only system that provides real-time notification to activate bomb shelters — travelers MUST download this before arrival. Israel has extensive bomb shelter infrastructure; compliance is a matter of life. Airlines have suspended; reduced; and restored Israel routes multiple times based on security conditions — verify flight status up to the day of departure.

Israel and the West Bank and Gaza are specifically named on the active Global Polio Travel Health Notice (CDC). Circulating poliovirus has been detected in environmental samples in Israel and the West Bank. The primary risk zone is the West Bank and Gaza. For Israel proper; risk to tourists is low but nonzero — environmental poliovirus has been detected in Israeli sewage surveillance. Confirm polio vaccine is up to date: adults who received primary childhood series should receive one adult booster if not previously given; travelers who have never been vaccinated require primary series.

Terrorist groups; lone-actor terrorists; and violent extremists continue to plot attacks in Israel; the West Bank; and Gaza. Attacks may target tourist locations; transportation hubs; markets; and local government facilities. The security environment is complex and can change without warning. IDF military operations are ongoing. Violence can occur anywhere in the country without advance notice. Demonstrations occur frequently and can become violent. Checkpoints in the West Bank require significant additional time and situational awareness.
Health Risk Callout
The polio situation is the clinical surprise for Israel that many travel medicine clinicians miss. Most physicians don't associate polio with Israel in their mental model — it's a high-income; technically sophisticated country. But the Global Polio THN specifically names Israel and the West Bank and Gaza; environmental poliovirus has been detected in Israeli sewage surveillance; and the West Bank and Gaza carry real transmission risk. Every Israel traveler needs a polio check: has this person received at least one adult polio booster? The security situation is the dominant counseling topic; but the polio question is the one that distinguishes a thorough pre-travel visit.

Israel's risk profile varies dramatically by geography. Tel Aviv and Jerusalem — the primary tourist destinations — are at Level 3 (Reconsider Travel) with rocket; missile; and drone threat. The entire northern border zone within 4km of Lebanon and Syria is Level 4 Do Not Travel due to continued IDF military presence and activity. Gaza is Level 4 Do Not Travel with all Erez and Rafah crossings closed. West Bank Area A (including Bethlehem and Jericho) is off-limits to US government employees. Southern Israel within 11.3km of the Gaza demarcation line is restricted. The Negev; Red Sea coast (Eilat); and central coastal areas have somewhat lower exposure but remain subject to rocket alerts during escalations. Haifa and the north coast carry elevated risk due to proximity to the Lebanese border conflict zone.

Israel's hospital system is world-class throughout the major cities. Hadassah; Sheba; Ichilov; and Rambam Medical Center in Haifa all represent genuine global excellence. Emergency care including trauma from blast and missile injury is an area of particular Israeli medical expertise developed over decades of conflict medicine. MAGEN DAVID ADOM (MDA) — Israel's emergency medical service — is highly professional and rapidly deployable. In the event of a rocket attack; hospitals have hardened units and operational protocols for mass casualty events that are among the most practiced in the world.

The clinical health picture of Israel is actually excellent: no malaria; safe tap water; world-class hospitals. Hadassah Medical Center on Mount Scopus is the kind of institution where I would want to be a patient. The security picture is what dominates this discussion.
The Red Alert app is the first thing I discuss — not because it's a medical intervention but because knowing about a rocket 90 seconds before it hits gives you time to get to a bomb shelter; and Israeli cities have bomb shelters in virtually every building. The thing most travelers don't realize is that Israel has developed extraordinary conflict medicine over decades — the trauma surgery and blast injury care at Sheba and Ichilov is genuinely world-leading. That expertise exists because it has to.

Know the location of the nearest bomb shelter (mamad) to your hotel; verify on check-in. In the event of a siren: move to a shelter or internal room; stay away from windows; wait at least 10 minutes after the last siren before leaving shelter. Avoid all demonstrations. Do not travel within 11.3km of Gaza or within 4km of Lebanese and Syrian borders. Confirm commercial flight status directly with your airline on the day of departure — Israeli airspace has been closed during escalations.
