Vaccine and Malaria Prevention

- Areas: Great Exuma island only. Nassau, Paradise Island, Freeport, Eleuthera, and the Exuma Cays themselves do not have established malaria transmission — the risk is specifically on Great Exuma island.
- Recommended Drug: For travelers to Great Exuma: atovaquone-proguanil, chloroquine, doxycycline, mefloquine, or tafenoquine (P. falciparum in the Bahamas is chloroquine-sensitive — Caribbean exception).

Malaria on Great Exuma is real but often completely unknown to travelers. The Exuma Cays are an increasingly popular yacht charter and boating destination — travelers chartering through the Exuma chain should specifically discuss malaria risk on Great Exuma with a travel medicine provider. This is a clinical detail that will surprise most travelers and most general practitioners.
Physician Note
The Bahamas has a clinical split personality. Nassau is a sophisticated city with Princess Margaret Hospital and Doctors Hospital — a real, modern emergency facility. If something goes wrong in Nassau, you will receive good care.
But Out Island travel changes the clinical equation completely. I always ask patients exactly which islands they're visiting. If the answer includes a yacht charter through the Exuma Cays, we need a conversation about malaria on Great Exuma, minimal medical care on the cays, and mandatory evacuation insurance.
The dengue and Zika risk is lower here than in Jamaica or the DR, but it's present — DEET at dawn and dusk is still standard advice. The clinical issue I counsel most specifically for the Bahamas is water sports safety. The jet ski sexual assault documentation is not anecdotal — it's documented in the State Dept advisory and the Embassy restricts its employees from independent operators.
That's a significant signal. The lesson: use resort-affiliated operators, verify credentials, and don't get on any boat with an operator who can't show you current safety documentation. Great trip destination when prepared correctly.

- If visiting Great Exuma specifically, consult a travel medicine physician about malaria prophylaxis

- Research all water sports operators in advance and identify resort-affiliated or licensed operators only
- If traveling to Out Islands confirm the location of the nearest clinic or hospital.
